authority to protect the health and welfare of TEXAS Medicaid and HHS
Departments. They prohibit the participation of any individuals or business, in
any Federal Healthcare programs if they are included on the Exclusion list. Generally,

to the patient safety or perform any fraud. This list of Excluded Individuals
